NEW ELIGIBILITY RULES

FOR ALL

FULL TIME AND PART TIME MEMBERS

 

The Trustees announce the following change and clarification in the Teamsters Local 671 Health Services and Insurance Plan regarding the Plan’s eligibility rules.

Effective January 1, 2008.

 

1.     INITIAL ELIGIBILITY

A.                For FULL-TIME participants in the Teamster Gold Plus Plan and the Teamster Gold Plan – To establish eligibility:

1)      The Fund Office must receive a total of at least 250 hours in contributions in two consecutive months.

2)      Your eligibility will start on the first day of the second month after your Employer has contributed a total of 250 hours in two consecutive months.  Example: You work at least 250 total hours in July and August (32 hours per week (x) 8 weeks = 256 hours) for which contributions are made to the Plan. You will become eligible on the first of October.

 

B.                 For PART-TIME participants in the Teamster Gold Plus Plan and the Teamster Gold Plan – To establish eligibility:

1)      The Fund Office must receive a total of at least 250 hours in contributions in four consecutive months.

2)      Your eligibility will start on the first day of the second month after your Employer has contributed a total of 250 hours in four consecutive months. Example: You work at least 250 total hours in July, August, September and October (3 ˝ hours per day (x) 5 days=17 ˝ hours per week, 17 ˝ hours per week (x) 16 weeks=280 hours) for which contributions are made to the Plan.  You will become eligible on the first of December.

 

2.     TERMINATION OF ELIGIBILITY

A.                For FULL-TIME participants in the Teamster Gold Plus Plan and the Teamster Gold Plan – Your eligibility will end when:

1)      The Fund Office receives less than a total of 384 hours in contributions in three (3) consecutive months.

2)      Your eligibility will end 3 months after your Employer has contributed a total of 384 hours in three consecutive months. Example:  The Fund Office received 384 hours for work performed in May, June, & July. But, the Fund Office receives less than 384 total hours for work performed in June, July and August. You would lose eligibility October 31.

3)    HOWEVER, IF YOU ARE LAID OFF DUE TO LACK OF WORK YOU MUST INFORM THE FUND OFFICE:

a)                  Your eligibility will terminate at:

            The end of the 4th week following:   

(i)         Any period of three consecutive months in which the Fund Office receives a total of at least 128 hours, and

(ii)        At least one month in that period is at least 64 hours.

(iii)       Example: Because you are laid off due to a lack of work, you  work 96 hours in March, 64 hours in April and 0 hours in May.  Your eligibility ends at the end of the fourth week in

4)      Call office for special rules, if you can not work because of disability.

5)      Call office for special rules, if you can not work because of a strike or lockout.

B.                 For PART-TIME participants in the Teamster Gold Plus Plan and the Teamster Gold Plan – Your eligibility will end when:

1)      The Fund Office receives less than a total of 250 hours in contributions in four consecutive months.

2)      Your eligibility will end two months after your Employer has contributed a total of 250 hours in four consecutive months.  Example: The Fund Office received 250 hours in May, June, July & August; but the Fund Office receives less than 250 hours in June, July, August & September.  Your eligibility would terminate the end of October.

 

 

BACK TO COLLEGE

Is your son or daughter back to college?

The Health Fund needs a letter from the College stating that your son or daughter is attending as a Full-Time Student in order to maintain eligibility on the Plan. The credit requirement for Full-Time status is 12 credits. Please have your son or daughter check with the College Registrar’s Office on how to obtain a letter confirming that he or she is enrolled as a Full-Time Student for the current semester. This letter is needed for the Fall Semester starting in August/September and again for the Spring Semester starting in January.

 

All Full-Time College Students over the age of 19 are eligible for their own ID Card from the Teamsters Local 671 Health Fund to keep with them when away at school. This ID Card entitles them to the benefits of the Plan. It is important for students to carry this card with them in case they need to obtain prescriptions or medical care.
