ELIGIBLE TEAMSTERS LOCAL 671 HEALTH SERVICES AND INSURANCE PLAN
RETIRED MEMBERS PLAN ONLY

The Trustees of the Teamsters Local 671 Health Services are in the process of implementing a NEW prescription pharmacy vendor effective AUGUST 1, 2008. We will no longer be using Caremark after August 1st for our prescription needs. This change is necessary for the future health of our Fund in the battle against the ever rising drug costs.

 

Effective August 1, 2008 all retired members that are currently receiving the Teamsters Local 671 Health Services and Insurance Retired Members Plan will receive prescription benefits through “Teamsters Rx”. Teamsters Rx is a non-profit mail-order pharmacy that is Teamster Union owned and operated by a Teamster Union staff in Manchester , NH . It also has a national network of 50,000 retail pharmacies, excluding Wal-mart and Sam’s Club.

 

The Retail guidelines will remain the same. You may use a participating retail pharmacy for an original prescription for up to a 30 day supply, but the Plan will pay up to $250.00 maximum in a calendar year per person after your $5 / $15 / $30 copay.  The mail order service must be used for medications requiring a refill, including all maintenance drugs. No refills are covered at retail pharmacies.

 

The Teamster Rx Mail-order pharmacy is in Manchester , NH . The mail-order copays will be the same, $10.00 for a generic and $25.00 for a brand name. A Brand Name will be dispensed ONLY when a Generic is not available. If a Generic is available, then a Generic will be dispensed. Do not have your doctor order DAW (dispense as written) or Brand name required.

 

Nexium, Prevacid, Protonix, Aciphex are not stocked at the Mail- Order pharmacy, Omeprazole (generic of Prilosec) is carried and dispensed.

 

The Health Fund will mail you a New ID Card that will be used for Prescription and Medical needs, a “Patient Profile” form asking for: Name, delivery address, home telephone number or cell phone number, email address, drug allergies. Also mailed to you will be a “Credit / Debit Card” form to pay the required co-pays.

 

You will need to get all new prescriptions from your doctors for all your medication needs to submit to Teamster’s Rx mail-order pharmacy when it is refill time. Caremark will NOT be sending your current prescriptions over to Teamster Rx to refill.

 

DO NOT SEND your new prescriptions to Teamster Rx until it is time to refill them. 14 days before your run out will be plenty of time.

 

The doctor can mail, fax, call, or e-prescribe prescriptions to the Teamsters Rx mail-order pharmacy. Patients also can mail new prescriptions. Note: patients may NOT fax their own prescriptions (this is a law). Teamster Rx prefers the doctor to fax, call or e-prescribe the original prescription, as the prescription will be filled that day and sent out in the next day’s mail. Ordering refills is as easy as calling Teamsters Rx mail-order at 1-866-888-0104, 24 hours a day / 7 days a week, follow the prompts and enter your prescription number into your telephone. Mailing in a refill is available but adds time to delivery.
